Health Tourism Services Informed Consent Form
This document has been prepared by us, Flymeditrust Travel Agency, a brand of [EDB Turizm Org.ve Dan. Hiz. Ltd. Şti.], due to our activities in accordance with the legal regulations on tourism, health tourism and consultancy services, and it contains details of services of intermediary institutions of Health Tourism which will provide “Healthcare Services” to our guests coming from abroad through contracted institutions and doctors, and all other services provided. In this regard, the importance and content of the services offered and the consent to be obtained are explained below:
1- Services Provided
Health Tourism Consultancy: We provide consultancy services to our guests coming from abroad in line with "health tourism" laws, and within the scope of this service we provide consultancy guidance about the selection of contracted health institutions, appointment arrangements, travel arrangements, and other matters on logistics, consultancy, reception, etc.
Contact with Health Institution, Facility and/or Doctor: We support our guests in selecting health institutions or doctors that can provide services that suit their needs, and help them with their appointments, information and transportation.
Accommodation and Transportation Arrangements: Upon the request of our guests, we make arrangements for transportation and accommodation in available facilities.
Coordination of Medical Treatment: We provide coordination during the medical treatment of our guests, help with communication to arrange their appointments, convey information about their processes as informed by the relevant health centers, and accompany them with guiding services in line with their requests.
Assistance on Legal Regulations and Documentation: During the health tourism process, we assist our guests by providing the necessary legal regulations and requested documents through our e-mail address: info@flymeditrust.com.
Personalized Remote or On-Site Reception Service: Upon the request of our guests, we provide a personalized reception service at the airport before their travel or at the arrival point. Within the scope of this service, we provide a reception service according to the arrival time of our guests, and all necessary logistic support is provided by Flymeditrust or procured from external real or legal entities.
Intermediary Service for International Travel Insurance: To protect our guests against possible health problems during their travels, we act as their intermediary for the purchase of international travel insurance. Within the scope of this service, the guests are offered insurance options suitable for their needs with up-to-date, instant data received from our service providers, and the necessary transactions are completed upon your written approval and service provider’s approval.
2- Cancellation, Prepayment and Healthcare Fee Refund: For the prepaid services, namely, health tourism and all other tourism services, presentations, information and all service purchases that fall under the scope of consultancy and intermediary institutions, the prepayment is not refundable even in cases where a consent form is not obtained, since we will have started to provide services. If the guests make a separate payment to the service provider for the healthcare service fee, the Refund/Cancellation Policies of the relevant institution and/or doctor will apply. If our guests do not pay the service fee, we will not provide any service for it and the prepayment fee will not be refunded.
3- Accuracy of Information We are not obliged to investigate the accuracy of the information, documents and medical reports on the current medical conditions requested from our guests within the scope of the health service they will receive, and we will not be liable for any damages suffered by the guests and/or other persons due to any incorrect information, documents and medical reports provided by the guests. Accordingly, we kindly request our guests to provide correct information, documents and medical reports.
4- Liability We are not liable for any disputes arising from the treatment, service or other relationships between the guests and the service-providing institution and/or doctor. In this regard, we are not liable for any material and/or moral compensation due to the dispute between the institution and/or doctor providing service to our guests.
Informed Consent:
In order to receive the above-mentioned services, our guests are required to give informed consent. This consent is obtained to ensure that our guests understand the details, risks and benefits of the services offered. Before giving the consent, our guests are given detailed information about our company's services, obligations and responsibilities. If they have any questions, they can ask them through our e-mail address info@flymeditrust.com.
Informed consent is important to ensure that our guests act consciously and safely throughout the health tourism process. Our company will act prudently to keep our guests' personal information confidential, to provide services in accordance with ethical rules and legal regulations, and to ensure their security. However, we would like to remind you that you should also ask for informed consent from the institutions or persons who will provide “Healthcare Service” to you.
This document intents to inform our guests about the services offered to them on [...../...../..........]. By signing this document, our guests declare that they understand the details of the services and that they are willing and giving consent to receive the services during the health tourism process.

"By contacting us through our website, social media, any digital platform, or directly via phone or by reaching our address, or through any other digital channels not listed here; you are deemed to have read and understood this Informed Consent Form and digitally accepted and consented to all the services we provide through digital platforms and health tourism services for yourself and your accompanying guests."
"For each health service evaluated within the scope of Health Tourism Services in Turkey, it is mandatory to have a separate 'Complication Insurance'. In accordance with this rule, during the service procurement process, if requested, you can obtain insurance from contracted and approved insurance companies by paying an additional service fee and insurance cost through a separate invoice. Afterwards, a copy or the original of your insurance will be shared with you. By refusing to obtain 'Complication Insurance', you acknowledge and undertake that all responsibility lies with you as the service recipient."
